COMMUNITY OUTREACH SUPPORT

Pathway Ltd offers community outreach support services for people living in their own home who need additional support. We assist our Service Users to develop and maintain as much independence and control as possible, whilst enhancing their quality of life. Our floating support services are carried out by experienced staff and are all DBS checked.

Our support services do not include personal care.

· Drug and Alcohol abuse

our community support packages include:

· Budgeting and managing finance

· Maintaining Tenancy

· Accessing welfare benefits (i.e filling out forms etc)

· Managing benefits, bills & savings

· Life skills 

· Accessing training, education & employment opportunities

· Registering with GPs, Dentist, and Opticians etc.

· Accessing specialist counseling services

· Social Inclusion

· Accessing recreational facilities

· How to keep safe in the community

· Building and maintaining positive relationships
